Medi-Cal Capacity Grant Program

Capital Program

Purpose

This program provides funding to support the construction, renovation and/or acquisition of medical facilities, mobile medical clinics and community-based and school-based wellness centers that will serve the Medi-Cal population in the Alliance service areas. Funding is also available for fixed asset equipment projects.

Current Status

Applications are currently only being accepted from Merced County.

Eligible Applicants

To be considered for Capital Program funding, applicants must meet the minimum eligibility requirements for the Medi-Cal Capacity Grant Program (see Eligibility Guidelines) and the program-specific requirements below.

  • Operate as a 501(c)(3) nonprofit or governmental entity that provides services to a significant volume of Medi-Cal members in the Alliance service area.
  • Be able to secure additional financial commitment to support the project from other sources such as donations, in-kind support, cash or documented loans or lines of credit, and/or other state, local or grant funding. See Funding Amount and Term section below for details on the percentage of project funding required.

For the project site of the construction, renovation or fixed asset equipment projects, demonstrate one of the following:

  • Ownership of land and/or facility, as applicable.
  • A secured long-term lease agreement for land and/or facility, as applicable.
  • Financing plans for the acquisition of the land and/or facility, as applicable and plans to operate the proposed site/asset to serve Medi-Cal members for at least 10 years after the project’s completion.

Types of Funded Projects

The Alliance provides Capital grants that can be used to support project costs associated with:

  • The construction, renovation or acquisition of facilities that provide medical services (including mobile medical clinics). Services include medical, behavioral health and substance use disorder treatment.
  • The construction, renovation or acquisition of facilities that serve primarily as community-based and school-based wellness centers.
  • The purchase of fixed asset equipment and related installation/construction.

Project costs may include pre-development activities. An applicant organization is limited to one Capital grant for one site-specific project. Organizations that have previously received an Alliance Capital grant are eligible to apply for funding under this current opportunity. All Medi-Cal Capacity Grant Program awards are one-time grants. Funding for ongoing maintenance and sustainability costs will not be considered.

Preference will be given to applications that best demonstrate how capital expansion would increase access to care and support the Alliance’s funding goals.

Capital grant awards will depend on the total number of applications received and amounts requested by each applicant. The Capital Program will continue to accept applications in subsequent rounds, contingent upon the availability of funds.

Examples of project components that could be funded:

Pre-Development
  • Capital project business plan addressing the rationale, scope and viability of a proposed expansion.
  • Financial projections for proposed and ongoing operations and maintenance.
  • Legal fees rendered for specific pre-development planning activities.
  • Architectural design and drawings.
  • Permits required to implement capital project.
Implementation
  • Capital grants can fund the acquisition or renovation of mobile medical clinics to serve Medi-Cal members in geographic regions where lack of transportation to services is a barrier to accessing care.
  • Construction or acquisition of community-based or school-based wellness centers to coordinate care and connect Medi-Cal members to services and resources.
  • Expansion or renovation of existing community-based or school-based wellness centers to coordinate care and connect Medi-Cal members to services and resources.
  • Fixed asset projects that play a crucial role in the organization's operations and long-term ability to effectively serve the Medi-Cal population (i.e., acquisition and/or installation of specialized machinery or equipment).
    • Examples of fixed asset equipment include a telemedicine platform, equipment for patient care, laboratory or imaging, or essential facility management equipment (e.g., HVAC, boiler, generator).

Funding Amount and Term

Amount

The Alliance will consider funding Capital grants up to 75% of total project costs, not to exceed a maximum grant award of $2,500,000. Applicants must demonstrate that the initial 25% of funding has been secured.

Funds can be used for capital expenditures only. Project budgets can include equipment and furnishings that are necessary for the delivery of care.

Funds cannot be used for rental subsidies, operating costs or services.

Funding will be considered for projects already in progress. However, funding will not be considered if activities are completed, or costs are incurred prior to approval of the grant request by the Alliance.

Timelines

Project timelines for Capital grants should not exceed 36 months. Project timelines are defined from the date the project scope of work begins to the date of project completion, inclusive of pre-development and implementation activities.

Capital grant awards will be paid in installments with the final installment distributed upon project completion.

Reports

Progress report(s) will be required to initiate installment payment(s) after the initial payment is made upon grant agreement execution. A final report is required one year after project completion on the impact of the funded project.

Applications require submission of financial statements, including:

  • Audited financial statements for the last two fiscal years.
  • An organizational profit and loss statement and balance sheet for the last 12 months.

If audited financial statements are not available, applicant should provide a rationale for not performing an annual audit and upload their company or organization’s most recent tax return or Form 990.
