Enhanced Care Management and Community Supports

Enhanced Care Management (ECM) and Community Supports (CS) are designed to be a set of intensive care coordination services that incorporate multiple delivery systems to provide a person-centered approach to care. These benefits support both the clinical and non-clinical aspects of care for the highest-need Medi-Cal members.

With the provision of ECM and CS being anchored in the community, community-based organizations (CBOs) and community partners play an integral role in the success of these programs. The goal of these services is to be delivered in an in-person manner by community-based providers, to the greatest extent possible. The design of the ECM and CS benefits is to acknowledge the importance of providers working together within a community to identify the needs of members and work towards a more holistic community approach to care. CBOs and community partners are involved not only in servicing members, but identifying members who may benefit from services, discussing new initiatives to support the programs, formulating plans for data sharing and identifying best practices to ensure members can successfully receive the full benefit of these services.

As new initiatives, ECM and CS have pushed the boundaries of a traditional health care delivery approach to begin incorporating the impact of Social Determinants of Health and acknowledge the importance of partnerships amongst local Medi-Cal managed care plans, CBOs and community providers to provide quality accessible care and support for improved health outcomes for the highest-risk members.

Funding for ECM/CS Infrastructure and Capacity Building

CalAIM Incentive Payment Program. Implementation of Enhanced Care Management and Community Supports requires significant investments in care management capabilities, infrastructure, information technology and data exchange, and workforce capacity at both the managed care plan and provider levels.

The DHCS CalAIM Incentive Program (IPP) is a component of CalAIM to support the development of a robust Medi-Cal network to deliver ECM and Community Supports services.  

  • The Alliance is administering this incentive funding to support our local provider network to address gaps and build infrastructure and capacity for ECM and Community Supports services.
  • IPP cannot overlap with other DHCS incentive funding for ECM and Community Supports, such as CITED (see below).
  • To facilitate incentive funding, the Alliance is intaking information from providers about their identified infrastructure and capacity needs via the ECM/CS Certification Tool.

Capacity and Infrastructure Transition, Expansion and Development (CITED) Initiative. CITED is a grant program administered by DHCS that provides funding to enable the transition, expansion and development of ECM and Community Supports capacity and infrastructure. CITED applicants must be contracted with a managed care plan for ECM and/or Community Supports services and are encouraged to coordinate applications with local organizations they contract with or intend to contract with to provide ECM and/or Community Supports services. Applicants who wish to receive CITED funding must submit an application indicating how they intend to use CITED funding. For more information, visit the DHCS page on CITED.
