Refer members to Enhanced Care Management and Community Supports

We’re excited to announce that we have provided Enhanced Care Management (ECM) services to 24,000 members since launching the program, with over 18,000 members currently enrolled. This is only possible through invaluable partnerships with our community partners! 

Enhanced Care Management (ECM) and Community Supports (CS) services coordinate both the clinical and non-clinical aspects of care for the highest need Medi-Cal members. Please join us in referring members to these important services! 

What are ECM/CS services? 

ECM and CS services assist eligible members in the following areas: 

  • Improve coordination of care between providers. 
  • Facilitate referrals and follow-up to community resources and services. 
  • Address social determinants of health. 
  • Improve health outcomes. 
  • Decrease inappropriate utilization and duplication of services. 
  • Help members navigate through complex systems and get connected to medical, behavioral health and social services.

How to refer members 

Anyone can refer members to ECM and CS services, including providers or a members’ family members. We use a “no wrong door” approach. You can refer someone for ECM and CS services by submitting a referral form. You can also encourage them to call Alliance Member Services at 800-700-3874 for support.
